The Household Pest Control, Prevention and Protection Policy

Each year during the term of this policy, AEREX PEST CONTROL SERVICES, INC. will visit the premises twice to treat and/or inspect for the control and prevention of ants, waterbugs, silverfish, spiders, crickets, roaches, centipedes, millipedes, drain flies, grain beetles, meal moths, rats and mice.** Within 30 days of each semi-annual visit, you may call for a re-treatment at no charge should the pest problem persist. To allow for maximum effectiveness, we request a two-week waiting period before scheduling any such re-treatment.

AEREX PEST CONTROL SERVICES, INC. further agrees to make call-backs between the semi-annual visits on request and treat for the above-mentioned pests at an additional charge of $35.00 per visit or $40.00 if billed. The 30-day re-treatment provision applies to all such call-backs.

AEREX PEST CONTROL SERVICES, INC. is fully covered with property damage and bodily injury insurance.

Treatment rates are $145.00 for the first eight rooms of the premises plus $3.00 for each additional room. Basements, attached garages, crawl spaces and attics are considered separate rooms. Closets, bathrooms, pantries and hallways are not considered separate rooms. There will be a $10.00 billing fee for any account not paid at time of service.

This policy remains in force for a period of one year and will then continue from year to year unless notice is give by either party to discontinue. If the policy is maintained without interruption, subsequent rates will be $10 less than the initial prevailing rate.

**In certain cases, pests may not respond to conventional treatments. This could necessitate extra work which would be performed by a service manager. In these cases, there will be no charge for materials. Labor charges will be billed at a rate of $35.00 for the first half hour and $15.00 for each additional quarter hour.
